The Greater Wisconsin Agency on Aging Resources, Inc.

The Greater Wisconsin Agency on Aging Resources (GWAAR) is one of three Area Agencies on Aging (AAAs) in Wisconsin, and supports local Aging and Disability Resources Centers (ADRCs) that provide programs and services for adults over the age of 60 as well as veterans and people with disabilities. GWAAR partnered with CCS Health™, implementing CTI+, to reduce readmission rates and health outcomes for clients completing the 30-day Care Transitions Intervention® program, increase reimbursement for payers, and demonstrate value to local referral organizations and health organizations.

Community CarePort HUB

The CarePort Community HUB is part of the Cascade Pacific Action Alliance (CPAA) based out of Washington state. CarePort partners with local agencies and trained community care coordinators that identify and reduce individual and household social determinants of health, primarily for Medicaid-eligible community members. They have partnered with CCS Health™ since 2018, and use our Fortify solution to track client records, manage referrals and Pathways to care, and create custom forms and reports to increase funding and reimbursement, among other features. In recent years, they were recognized for completing the most Housing Pathways of any agency using the CCS platform nationwide.

Harris County HUB

In 2019, the Network of Behavioral Health Providers – Houston launched the C3 Pilot in the Houston, Texas area, focusing on zip codes shown to have the highest mental health needs. In the same year, a coalition of social, primary, and behavioral health organizations launched an initiative called Healthy Woman Houston (HWH), aimed at addressing maternal mortality and morbidity in the zip codes in Houston and the surrounding area with the highest mortality rates for women who had recently given birth.
